GLP-1 medications cause thyroid cancer in humans

Verdict

Mostly false

A kernel of truth, but the headline claim is misleading.

Short answer: The FDA black-box warning is based on rodent studies. Human data has not shown elevated medullary thyroid cancer risk in clinical trials or post-market surveillance, though personal/family history of MEN-2 or medullary thyroid carcinoma remains a contraindication.

The FDA boxed warning on Wegovy, Ozempic, Mounjaro, and Zepbound — about thyroid C-cell tumors — comes specifically from rodent studies where rats given high-dose GLP-1 RA developed thyroid C-cell tumors. Whether this translates to humans is the actual question.

Multiple post-market surveillance studies in humans, including a 2024 JAMA analysis of >170,000 GLP-1 RA users, have not detected elevated medullary thyroid carcinoma incidence vs matched controls. The relative risk in the most-cited cohort (Bezin et al., 2023) appeared elevated but was confounded by surveillance bias (GLP-1 users get more screening).

What is actually contraindicated

  • Personal history of medullary thyroid carcinoma
  • Family history of medullary thyroid carcinoma
  • Multiple Endocrine Neoplasia type 2 (MEN-2) syndrome

For patients without these specific risk factors, current evidence does not support avoiding GLP-1 medications on thyroid cancer grounds alone. The rodent-data warning persists on labeling because regulatory caution outweighs ambiguous human evidence, not because the human signal has materialized.
