Brain fog: treatments compared (2026)

Cognitive fog in perimenopause is real and usually transient — HRT helps subset of cases.

What it is

Mid-life cognitive complaints (word-finding, slower processing, memory lapses) peak in perimenopause + early postmenopause. SWAN study confirms measurable cognitive dip during the transition that usually resolves postmenopause.

Who experiences this

Perimenopause is the peak period. Sleep loss + mood changes amplify perceived severity. Resolves in most by 2-3 years post-final-period.

Treatment options compared

All options below are real published treatments. Evidence-level reflects current clinical guidelines (NAMS / ACOG / Endocrine Society). Cost ranges are starting price estimates from $US public sources.

Systemic estrogen (early postmenopause window)

HRTsecond lineFDA-approved

Monthly cost

$25-$150

Side effects

breast tenderness, spotting

Modest improvement in verbal memory in early postmenopause per KEEPS study. Effect smaller than for VMS. Not first-line for cognition alone but and bonus benefit if VMS also present.

Monthly cost

$0-$200

Side effects

minimal

Fixing sleep deprivation + treating night sweats often resolves brain fog without targeting cognition directly.

Aerobic exercise

Lifestyleadjunct

Monthly cost

$0

Side effects

minimal

150 min/week moderate-intensity aerobic exercise improves executive function in midlife women per multiple RCTs.

When to see a clinician

  • Cognitive changes are progressive (vs episodic / "fog")
  • Family member notices issues you donot
  • Younger than 45 with cognitive complaints (rule out other causes — thyroid, B12, sleep apnea)

FAQ

Will HRT prevent dementia?

Current evidence is mixed. KEEPS-Cognitive Trial showed neutral cognitive effects of HRT in early postmenopause. WHIMS (older women, higher dose) showed harm. Current consensus: starting HRT early for VMS may have small cognitive benefit; not start HRT solely to prevent dementia.
