Why menopause may cause bloating
Fluctuating estrogen and progesterone alter gastrointestinal motility and fluid retention. Progesterone slows smooth-muscle contraction, which delays gastric emptying and colonic transit, while estrogen surges cause water retention in soft tissue. The result is abdominal distension that often feels worse than premenstrual bloating.

Roughly 30 to 40 percent of perimenopausal women report new or worsened bloating. Symptoms usually peak in the two years around the final menstrual period. Persistent bloating combined with early satiety, weight loss, or unexplained pain warrants pelvic imaging because ovarian cancer often presents this way in the same age band.

Treatment options
Low-FODMAP dietary trials, magnesium citrate for constipation, and simethicone for gas are practical starting points. HRT commonly improves bloating driven by hormone fluctuation. Persistent bloating requires a workup for SIBO, celiac disease, and pelvic pathology.

- Is menopausal bloating the same as PMS bloating?
- It stems from the same hormonal mechanisms but is more erratic. Perimenopausal cycles produce larger estradiol surges than reproductive cycles, so bloating can be more severe.
- When should I be worried about ovarian cancer?
- Persistent bloating for more than three weeks combined with early satiety, weight loss, or pelvic pain warrants transvaginal ultrasound and CA-125.
- Does HRT help bloating?
- HRT often stabilizes symptoms once cycling stops. Oral progestin can transiently worsen bloating in the first month.
- Are probiotics useful?
