Why menopause may cause night sweats
Falling estrogen sensitizes KNDy (kisspeptin-neurokinin B-dynorphin) neurons in the hypothalamic thermoregulatory center. The set point becomes narrower and more reactive, so small increases in core temperature during sleep trigger disproportionate vasodilation and sweating, followed by chills as the body overshoots correction.
How common is this?
Roughly 60 to 80 percent of women experience night sweats during the menopause transition, and about one-third experience severe episodes. The Study of Women’s Health Across the Nation documented a median duration of 7.4 years, with African American women experiencing symptoms for a decade or longer on average.

Treatment options
HRT is the most effective treatment and reduces frequency by 75 percent within four weeks. Fezolinetant is a first-in-class NK3 antagonist that specifically targets the KNDy pathway and is FDA-approved as of 2023. Low-dose paroxetine (7.5 mg), gabapentin, and oxybutynin are non-hormonal alternatives.
