- Continue Wegovy 1 week after last dose dateWegovy weekly dosing — Zepbound also weekly. Maintain prior week schedule.
- Start Zepbound at low doseBegin Zepbound 2.5 mg weekly, regardless of Wegovy dose. Titration restarts.
- Titrate per Zepbound schedule2.5 mg → 5 mg at week 5 → 7.5 mg at week 9 → 10 mg if needed. Side effects often less because slow titration.
- Reassess weight loss at 12 weeks5%+ loss at 12 weeks predicts continued response. If <5%, reconsider therapy.
