Three factors: estrogen-mediated fat redistribution from hips to abdomen, ~1%/year muscle loss from sarcopenia, and 30-50% drop in insulin sensitivity. Same calorie intake = more weight.

Estrogen receptors on adipocytes shift fat storage patterns. Sarcopenia accelerates without resistance training. Insulin sensitivity decline means carbohydrates more easily stored as fat. Average gain 1-2 lbs/year. Reversal requires resistance training (3×/week), protein intake (1.2-1.6 g/kg), and sometimes GLP-1 medications.
