Estradiol is the primary bioidentical estrogen used in menopausal hormone therapy. Available as transdermal patch, gel, oral tablet, vaginal cream, and vaginal ring. Transdermal routes avoid first-pass liver metabolism and carry lower VTE risk than oral. First-line per NAMS 2022 for vasomotor symptoms in women under 60.
