Estrogens carry an FDA class boxed warning covering endometrial cancer (with unopposed estrogen), cardiovascular disorders, breast cancer, and probable dementia, based largely on the Women's Health Initiative of oral conjugated estrogens. The transdermal route lowers clot and stroke risk relative to oral, but the class boxed warning still appears on patch labels. Treatment is individualized with a clinician.
