Estrogen products carry an FDA class boxed warning (endometrial cancer, cardiovascular disorders, breast cancer, probable dementia) based on systemic estrogen data from the Women's Health Initiative. It appears on the vaginal cream label as class labeling even though low-dose vaginal use produces minimal systemic absorption; use the lowest effective dose for the shortest appropriate duration.
Incidence rates from clinical trials. Individual experience varies.
| Effect | Incidence | Note |
|---|---|---|
| Vaginal discharge or wetness | Common | Partly the cream base itself |
| Local irritation, itching, or burning | Common | Usually mild and settles with continued use |
| Light spotting | Early treatment | Report any persistent or new postmenopausal bleeding to a clinician |
| Breast tenderness | Uncommon | Systemic absorption is low at maintenance doses |
| Headache | Uncommon |
Less common but require medical attention.
